Stronger Together: Bridging Pediatric to Adult Care is a dynamic, community-centered wellness and transition experience hosted by The Heart of Gold Sickle Cell Foundation of Northern Virginia. Taking place at the Chinn Center in Woodbridge, this event will bring together healthcare providers, Medicaid representatives, insurers, community advocates, mental health professionals, dental providers, and families to support adolescents and young adults living with sickle cell disease as they prepare to transition from pediatric to adult care.
